Old Kings Highway Application Forms

All development projects located north of Route 6 require Old Kings Highway (OKH) Historic District Committee review. All projects require a filing and the applications are as follows:

Certificate of Appropriateness
Submit this application for new construction, additions, exterior building changes, exterior painting with color changes, sheds, and signs.

Certificate of Exemption
Submit this application when the proposed activity is not visible from a way or public place or when the activity has been specifically exempted by the regional commission.


Certificate of Demolition or Removal
Submit this application for the demolition or removal of a building or structure or part thereof.

COVID-19 Update

  • Town staff is available by email. Beth Vozella, OKH Office Administrator, can be reached at bvozella@yarmouth.ma.us 
  • Application materials and associated timeframe waiver forms are available online as fillable PDFs (see links above). These materials can be filled out online by clicking on a line in the document and typing in information. Once completed you can save the filled out document and submit it as an attachment to Beth Vozella via email.
  • Applications can be received by email and be reviewed to the extent possible for completeness. In the event that file attachments exceed email size limitations, please utilize a file sharing feature such as DropBox or Google Drive to submit your materials electronically. 
  • Applicants will be notified electronically when applications are deemed “complete”.
  • Hard copy applications, or (required) hard copies submitted in conjunction with an application submitted by email, may be submitted by mail and will be reviewed when possible. 
  • Hard copies may also be dropped off at town hall in the designated dropbox location outside the entrance next to the side parking lot. Please be advised that hard copies will not be reviewed for three business days following drop off per order of the Board of Health.
